Pathways Connects with Weld County Leaders at Community Leadership Breakfast
Building strong community partnerships starts with bringing people together, sharing information and creating opportunities to learn.
Yesterday, Pathways Hospice & Palliative Care hosted a Weld Leadership Breakfast at FMS Bank in Greeley, welcoming community leaders for a morning of conversation and education about the healthcare services and support available to individuals and families throughout Northern Colorado.
Sharing Information About Care and Support
During the breakfast, attendees learned more about three important areas of Pathways' services: PACE, hospice and palliative care.
Each program offers a different type of support, helping individuals and families navigate healthcare needs, manage serious illnesses and maintain the best possible quality of life.
The conversation provided an opportunity to explore how these services work, who may benefit from them and how community members can connect with resources when they or someone they love needs additional support.
Stronger Connections, Stronger Communities
Community leaders play an important role in helping residents understand the resources available to them. By sharing information and building relationships with local organizations, Pathways hopes to make it easier for individuals and families to find the care and support they need.
We are grateful to everyone who took time out of their busy schedules to join us, ask questions and learn more about Pathways. A special thank-you to FMS Bank for hosting the gathering and helping bring our community together.
We look forward to continuing these conversations and strengthening the partnerships that help us serve our Northern Colorado neighbors!
