Pathways Hospice Community Care for Northern Colorado

Community education

A local non-profit, we dedicated ourselves to serving everyone in need of hospice care or grief support within our coverage area. As part of our services, we offer education and awareness programs as well as consultation and support to businesses, organizations and schools.

Community outreach programs includes:

  • group presentations
  • community involvement
  • printed educational materials
  • educational booths at community events

Need an excellent speaker for your group or business? Pathways Hospice can provide a speaker to educate your group on the basics of hospice care, pain management, signs and symptoms of dying, healthy grieving, volunteering or Five Wishes.

For business, organizations and schools, our grief counselors can also provide consultation for developing grief support response plans as well as debriefing and support following the loss of a co-worker or student. Learn more about our offerings through the links below.

Pathways Hospice serves Larimer County and Western Weld County in Northern Colorado which includes Fort Collins, Loveland and Windsor.
We offer Community Care that includes At Home Assistance for the Elderly, Sick and the Terminally Ill along with Grief and Loss Programs.
