Pathways Hospice Community Care for Northern Colorado

About Pathways Hospice care

Pathways Hospice offers full-spectrum hospice care, 24 hours a day, 7 days a week, every day of the year. Our specialized teams provide comfort, expert medical care, pain management, and emotional and spiritual support tailored to each patient's needs and wishes. Most care is provided in homes, but we also provide care in skilled nursing facilities, assisted living facilities, hospitals and any place you would call "home."

At the center of our philosophy of hospice care is the belief that each of us has the right to be pain-free and to live and die with dignity. To achieve this, we provide holistic care, including providing support to friends and family.

Our services include

  • On-call, 24/7 nursing care
  • Medical equipment and supplies related to the diagnosis
  • Personal care provided by Certified Nurse Aides (CNA)
  • Social services and counseling
  • Inpatient care
  • Medical oversight by Board Certified hospice & palliative care physicians
  • Pathways: Programs for Grief and Loss
  • Volunteer services
  • Spiritual care through chaplains
  • Creative therapies, including music and art therapy
  • Community education

In addition to regularly scheduled care, our staff will answer questions over the telephone or make emergency visits as needed. To access our care at any time of day or night, call our main number, 970.663.3500. When the office is not open, our answering service will connect you directly with the resources you need.
