We work closely with healthcare professionals in hospitals, assisted living and nursing facilities, providing support and consulting as well as complementary services.
Referral and intake

We keep trained admissions staff immediately available to you to facilitate smooth and timely transition to hospice care, both from a healthcare and an administrative standpoint. Upon receiving your referral we send out an admissions team within a four-hour time frame.
Communications

We initiate immediate and ongoing communication with patients, patients' families and care providers. Knowing that patients' situations can change from moment to moment, we want to make certain you always have the information and resources you need.
Resources

Pathways Hospice offers comprehensive resources to help you serve your patients and their families:
- Medical professionals. Our experienced medical
professionals, specialized in hospice care and grief support,
are available 24/7 for consultation and assistance.
- Grief support. Our PATHWAYS Programs for Grief
and Loss support not only hospice patients but also their families
and friends.
- Education. Our educators provide in-service
trainings on topics in hospice care and grief support.
- Complementary services. Our volunteers offer
a host of services, ranging from companionship and pet visits
to photography, tax preparation and massage therapy.
- Library. Our library and on-line resources
provide easy access to specialized information for patients, families
and care providers.
