Our care model

The premiere hospice provider in our community for three decades, Pathways Hospice offers the most comprehensive hospice and bereavement care available. Our experienced staff includes physicians Board Certified in Hospice and Palliative Care, nursing professionals, certified nursing aides, social workers, counselors, music therapists, massage therapists, chaplains and volunteers. Our staff understands the unique nature of dealing with terminal illness, and we have specialists on-call 24 hours a day, 7 days a week.

Essential to working together with other healthcare professionals, we prioritize

  • Serving as an information resource to which you may refer patients and families
  • Facilitating efficient, seamless transition to hospice services
  • Providing you with specialized knowledge regarding hospice, palliative and bereavement care
  • Maintaining clear, effective ongoing communication
  • Providing beneficial services, such as companionship or music therapy, which may fall outside the scope of services at a patient’s place of care

We offer Community Care that includes At Home Assistance for the Elderly, Sick and the Terminally Ill along with Grief and Loss Programs.
