Pathways Hospice Community Care for Northern Colorado

Pathways Hospice has been the premiere hospice provider in our community since 1978, We offer the most comprehensive hospice and bereavement care available in Larimer and Western Weld counties.

Our experienced staff includes physicians Board Certified in Hospice and Palliative Care, compassionate nursing professionals, social workers and chaplains, music therapists, and specially trained volunteers.

We partner with you to offer full-spectrum hospice care with staff who are on-call, 24 hours a day, 7 days a week, every day of the year.

You have access to our expert services that include:

  • Physician services:
    • Our Medical Director is Board Certified in Hospice and Palliative Care
    • Available for consultation regarding hospice, palliative, and bereavement care
    • Maintaining clear, effective, ongoing communication.
  • Expertise in home infusion therapy for resistant pain symptoms/issues
  • Nationally recognized Grief and Counseling services for your patients and their families
  • Serve as a resource to which you may refer patients and families
  • Companionship and respite care for caregivers by hospice-trained volunteers

We offer Community Care that includes At Home Assistance for the Elderly, Sick and the Terminally Ill along with Grief and Loss Programs.
