Referrals and intake
Pathways Hospice works closely with healthcare professionals in hospitals, assisted living and nursing facilities. To facilitate smooth and timely transition to hospice care—from both a healthcare and an administrative standpoint—we keep trained admissions staff immediately available to you. Upon receiving your referral we send out an admissions team within a four-hour time frame.
