Pathways Hospice Community Care for Northern Colorado

Pathways Counseling

Pathways Hospice offers individual, couples and family counseling. Counselors are licensed professionals with specialized training and experience in grief and loss. Because people grieve in different and uniquely personal ways, Pathways offers counseling to meet the distinctive needs of different age groups and types of loss. To support the needs unique to losing a parent, spouse, child, friend or co-worker, Pathways offers specific services and programs themed on these relationships.

Pathways for Grief and Loss additionally offers:

  • Art Therapy for adults and children
  • Play Therapy for children
  • Sand Tray Therapy for children

Fees and Insurance

  • Family members of Pathways Hospice patients are provided with their first six counseling sessions at no charge (additional sessions are available).
  • Community counseling clients
    • Sliding scale fee based on household income
    • Employee Assistance Programs (EAP) through your workplace
      • Cigna
      • Aetna
      • United Behavioral Health
      • ComPsych
      • Employee Assistance Service (Poudre Schools)
    • Medicare/Medicaid on a case by case basis

For more information on eligibility or to schedule an appointment, check with your employer's Human Relations department.

Pathways for Grief and Loss is not a crisis center and does not have 24 hour on call availability for counseling clients. Clients in a crisis situation are encouraged to call 911 or go to the nearest emergency room.

Pathways Hospice serves Larimer County and Western Weld County in Northern Colorado which includes Fort Collins, Loveland and Windsor.
We offer Community Care that includes At Home Assistance for the Elderly, Sick and the Terminally Ill along with Grief and Loss Programs.
